From showing how to monitor blood sugar and how to reduce stress, to setting goals for improved diet and exercise, the Seton Diabetes Education Center offers comprehensive education and support services for people diagnosed with diabetes. The program teaches how to avoid complications of diabetes and how to deal with them if they do occur. Support groups are available for the patient and family, as well as individual counseling for patients with special needs. Programs include:
Type 2 Diabetes Program
This program consists of an individual assessment and three interactive group classes taught by a registered nurse and registered dietitian. The focus is on behavior change, goal setting, healthy eating, exercise, medication and use of self blood glucose testing in management of diabetes. Information is provided in all other areas of diabetes self-management and prevention of long term complications.
Type 1 Diabetes Program
This program is provided by a registered nurse and registered dietitian and includes an individual assessment and instruction on a one-on-one basis. It covers all areas of diabetes self-management including different methods of insulin administration. Prevention of hypoglycemia, ketoacidosis and long term complications is also emphasized. The focus is on the use of self-blood glucose testing to assist the individual in making daily decisions regarding insulin, food intake and physical activity in order to maintain safe blood glucose levels.
Gestational Diabetes Program
This program consists of an individual assessment and a group class taught by a registered nurse and registered dietitian with one follow-up session. The focus is on healthy eating, exercise, self-blood glucose testing and oral medication/ insulin (if required). Strategies to prevent developing diabetes later in life are also emphasized.
Pre-Diabetes Program
This program consists of an individual session with a registered dietitian. The focus is on behavior change, healthy eating, exercise and possibly self blood glucose testing to reduce the risk or delay the onset of diabetes.
